To participate in camp it is required that each camper print the Medical Release Form and Medical History Form and bring the signed form to camp.
The release and the treatment authorization must be signed by a parent or guardian if the camper is under 18 years old. Campers who are 18 years old or will become 18 years old before the end of the camp/clinic must also sign. In order for campers to participate in camp activities, we must have this form prior to the camp’s start date. Otherwise, a parent or guardian must be contacted prior to release to participate.

Staffing
A full-time staff member will supervise camp participants at all times. Wrestlers will be required to attend all activities and comply with all camp rules and regulations. We provide one instructor per 15-20 kids and Kevin Jackson instructs at all camps. (Camp rules will be included in your confirmation packet.)
Registration
Each applicant must complete the appropriate registration form either online (recommended) or print off the online brochure and fill it out and send it in. If you need us to send you a brochure, please contact us. You will receive additional camp information (a confirmation packet) after the application has been submitted and a down payment or payment in full has been processed. The balance for each camp is due before the camp start date. Resident campers are asked to register at least three weeks before the start date of camp (we need to know the number of rooms we will need in the dorms). The price of each camp will increase 15% within three weeks of the camp start date due to housing contracts.
Camp Deposit
A $100 minimum deposit is due to hold your spot in camp. Once we receive your deposit or payment, we will send you a confirmation packet either via email or mail (you choose during registration). Your remaining balance must be paid before the camp start date (you can pay your remaining balance at check-in).
Returned Checks
There will be a $20 Charge for all returned checks. Please remember that the $100 Key Deposit Checks will be cashed and $100 Cash will be returned at the end of Camp when the key is returned.
Check-In/Check-Out
Check-in & check-out times and locations will be provided in your confirmation packet once we have processed your registration materials and receive at least your down payment. Campers will be responsible for their own dinner on the first day of camp and their own lunch on the last day of camp. Transportation to and from camp will be the camper’s responsibility. Airport pick-up is available upon request for a $30 fee payable at check-in; a travel itinerary must be on file with us one week prior to the camp start date. Early afternoon arrival times are suggested and greatly appreciated. There is a $100 key deposit due at check-in; this deposit will be refunded to the camper at check-out when the key is returned. Key deposits in the form of a check will be cashed, and the key deposit will be returned in cash at check-out.
Housing
Resident campers will be housed in air-conditioned dormitories (if possible). Your confirmation packet will detail the dorm location and check-in location. Your confirmation packet will also inform you on what will be supplied in the dorms and what is needed (ex. linens, towels, etc.). We will provide 24 hour supervision for all campers staying with us.
Laundry Service
(Strongly Recommended) A $30 laundry service is available to the 21 Day and 10 Day Intensive Training campers only. You will get your workout clothes cleaned each day by members of our staff. This service will be available at check-in and is strongly recommended to help control sanitation issues.
What to Bring
Each wrestler must bring suitable wrestling camp equipment (shorts, t-shirts, sweats, wrestling shoes, running shoes, casual clothes, towels, anti-bacterial soap, shampoo and an alarm clock). Don't bring things that will distract you from focusing on getting better. Your confirmation packet will further discuss things needed for camp.
Cancellations and Refunds
Administrative commitments require a nonrefundable deposit. If an application is withdrawn 21 days or more before the start of camp, you will receive a refund less $100. No refunds will be issued within 7 days prior to the first day of camp.
Health Insurance
The parent/guardian’s personal or injury insurance policy will be utilized as the primary insurance for the treatment of injuries and hospitalization for illness or injuries incurred during Kevin Jackson's Iowa State Wrestling Camps. A certified trainer will be on hand at all times. Forms to be completed will be included in confirmation packet.

Physician’s Authorization
Each camper must be cleared by a physician to participate. This is to certify that this individual was examined to be physically able to participate in vigorous physical and competitive athletic sports. School physical forms will be acceptable if valid within one year of the starting date of camp. Form will be included in confirmation packet.
Release of Liability, Medical and Surgical Authorization
In consideration of Kevin Jackson’s Iowa State Wrestling Camps and granting the student permission to participate in Kevin Jackson's Iowa State Wrestling Camps, I hereby assume all risks of his or her personal injury (including death) that may result from any camp Activities. As guardian, I do hereby release Kevin Jackson's Iowa State Wrestling Camps and their officers, employees, agents, all instructors, and all participants in Kevin Jackson's Iowa State Wrestling Camps from all liability, including claims and suits of law or in equity, for injury, fatal, or otherwise which may result from the student taking part in Kevin Jackson's Wrestling Camp activities. In addition, I hereby authorize and give my consent to the health authorities of Iowa State University or any licensed health professional to perform upon or administer any reasonable, necessary surgical or medical treatment. I also give permission to administer whatever anesthetic may be necessary or advisable during the medical or surgical procedures. This authorization is intended to cover emergency treatment, immunizations, injections, and minor operations and procedures. In the case of psychiatric and/or psychological treatment, parent authorization for treatment beyond that responsive to the emergency will be requested. I agree to assume all costs related to such treatment. I authorize my insurance company to pay benefits to Iowa State University Health Service or other hospitals and clinics.
Also, I authorize the disclosure of medical information to my insurance company for purpose of claim. I understand that I will be responsible for any medical or other charges in connection with student’s attendance at this camp. (Each camper must provide his/her own medical insurance.)
